About

The program provides up to $2,500 a month for Chickasaw elders to live in a state licensed assisted-living facility. The benefit is paid directly to the facility and is based on client’s need. Required application is available at the above address.

All applicants will be assessed to determine medical and financial eligibility. If approved for the program, applicant will be notified by program manager.

  • Chickasaw citizenship card
  • Proof of income
Eligibility
  • Must be Chickasaw
  • 60 years of age or older
  • Verified financial need
